Terms of Service

These Terms of Service govern your use of the Comprehensive Recovery Solutions website and telehealth services. By accessing our website or using our services, you agree to be bound by these terms. Please read them carefully before proceeding.
Section 01

Acceptance of Terms

By accessing or using the website at comprehensiverecoverysolutions.com or by using any telehealth services provided by Comprehensive Recovery Solutions (“CRS Recovery,” “we,” “us,” or “our”), you (“Patient,” “User,” or “you”) agree to be legally bound by these Terms of Service and all applicable federal and Florida state laws.
 
If you do not agree to these Terms of Service in their entirety, you must immediately discontinue use of this website and our services. These Terms of Service apply to all visitors, patients, and users of our website and services.
Agreement at start of treatment
By completing our pre-screening form, scheduling an appointment, or beginning treatment with CRS Recovery, you confirm that you have read, understood, and agreed to these Terms of Service in full.
Section 02

Description of Services

Comprehensive Recovery Solutions (“CRS Recovery”) is a Florida-licensed telehealth medical practice providing medication-assisted treatment for opioid use disorder. CRS Recovery provides the following services exclusively to adults aged 18 and older who are physically located in the state of Florida at the time of service:
  • Clinical evaluation for opioid use disorder (OUD) via secure telehealth video appointments
  • Prescribing of Suboxone (buprenorphine/naloxone) and other FDA-approved medications for the treatment of opioid use disorder, where clinically appropriate
  • Ongoing medication management and follow-up telehealth appointments
  • Supportive clinical follow-up and recovery-oriented care
  • Referral coordination with other healthcare providers when clinically indicated
CRS Recovery does not provide emergency medical services, inpatient treatment, detoxification programs, or residential rehabilitation. We do not accept insurance for clinical services. All services are provided on a self-pay basis.

Geographic limitation

Services are available only to patients who are physically located in Florida at the time of each telehealth appointment, consistent with applicable state licensure requirements.
Section 03

Eligibility Requirements

To be eligible to receive services from CRS Recovery, you must meet all of the following requirements at all times during treatment:
  • Be 18 years of age or older
  • Be physically located in the state of Florida at the time of each telehealth appointment
  • Be seeking treatment for opioid use disorder
  • Have access to a device with audio and video capability and a stable internet connection for telehealth appointments
  • Not be experiencing an immediate psychiatric or medical emergency at the time of service
  • Be willing to participate in telehealth appointments and follow your provider’s treatment recommendations
  • Provide accurate, complete, and truthful information to your provider at all times

 ⚠ Ineligibility

CRS Recovery reserves the right to decline services to any individual who does not meet eligibility requirements, provides false or misleading information, or whose clinical needs exceed the scope of our telehealth services.

Telehealth Services & Controlled Substance Prescribing

Legal authority to prescribe via telehealth

CRS Recovery provides telehealth services under the authority of Florida Statute § 456.47 and in compliance with federal regulations governing telehealth prescribing of controlled substances. Suboxone (buprenorphine/naloxone) is a Schedule III controlled substance under the Controlled Substances Act and is regulated by the U.S. Drug Enforcement Administration (DEA).
 
Florida law expressly permits telehealth prescribing of Schedule III through V controlled substances, including buprenorphine, for the treatment of opioid use disorder. CRS Recovery operates under current DEA telemedicine flexibility regulations, which authorize DEA-registered practitioners to prescribe Schedule III-V controlled substances via telehealth without a prior in-person evaluation, through December 31, 2026. These regulations are subject to change.
Important — regulatory notice
Federal telehealth prescribing flexibilities for controlled substances are subject to extension or modification by the DEA and HHS. If these regulations change, CRS Recovery may be required to modify its prescribing practices, which could affect your treatment. We will notify patients of any material changes that affect their care.

Prescription requirements

All prescriptions issued by CRS Recovery are subject to the following conditions:
  • Prescriptions are issued only after a thorough clinical evaluation by a licensed, DEA-registered provider
  • Prescriptions are issued for legitimate medical purposes in accordance with the standard of care
  • Suboxone prescriptions are issued only to patients with a clinical diagnosis of opioid use disorder
  • Prescription refills require ongoing participation in follow-up telehealth appointments
  • CRS Recovery reserves the right to modify, suspend, or discontinue prescribing at any time based on clinical judgment

Prescription drug monitoring

As required by Florida law and federal regulations, CRS Recovery reviews Florida’s Prescription Drug Monitoring Program (PDMP) — also known as E-FORCSE® — before prescribing controlled substances. By receiving services from CRS Recovery, you consent to our provider accessing your PDMP records as permitted by law.

Medication limitations

CRS Recovery prescribes Suboxone (buprenorphine/naloxone) for the treatment of opioid use disorder. We do not prescribe methadone for OUD treatment (methadone for OUD may only be dispensed at federally certified opioid treatment programs). We do not prescribe Schedule II controlled substances via telehealth except as permitted by Florida law.
Section 05

Payment & Billing

Self-pay only

CRS Recovery is a self-pay practice. We do not accept health insurance, Medicare, or Medicaid for clinical services. All fees must be paid prior to or at the time of your appointment.

Current fees

  • Initial assessment:$300 — includes clinical evaluation, diagnosis, treatment planning, and Suboxone prescription where clinically appropriate
  • Ongoing monthly care:$150 per month — includes follow-up appointments, medication management, prescription refills, and dosage adjustments
Fees are subject to change. CRS Recovery will provide advance notice of any fee changes to current patients.

Medication costs

Prescription medication costs (Suboxone) are billed separately by your pharmacy and are not included in CRS Recovery’s service fees. Many health insurance plans, including Medicaid, may cover the cost of Suboxone medication at the pharmacy. CRS Recovery does not control or guarantee pharmacy pricing.

Cancellations and no-shows

Please provide at least 24 hours’ notice if you need to cancel or reschedule an appointment. CRS Recovery reserves the right to charge a cancellation or no-show fee for appointments cancelled with less than 24 hours’ notice. Repeat no-shows may result in discharge from the practice.
No surprise billing
CRS Recovery will always confirm fees with you before beginning treatment. You will never be charged for a service you did not agree to in advance.
Section 06

Patient Responsibilities

As a patient of CRS Recovery, you agree to the following responsibilities:

Accurate information

  • Provide complete, accurate, and truthful information about your medical history, current medications, substance use, and any other information relevant to your care
  • Notify your provider immediately of any changes to your health status, medications, or circumstances that may affect your treatment
  • Never provide false information to obtain a prescription — this is a federal crime

Treatment participation

  • Attend all scheduled telehealth appointments on time and fully prepared
  • Take medications only as prescribed and never share, sell, or transfer your Suboxone prescription to another person
  • Notify your provider if you relapse or use any substances outside your treatment plan — this information will not result in immediate discharge and is essential for your safety
  • Participate in urine drug screening when requested by your provider as a standard condition of treatment

Technology requirements

  • Ensure you have a reliable internet connection, a functioning camera and microphone, and a private space for your telehealth appointments
  • Do not participate in telehealth appointments while driving, operating machinery, or in a public location where you cannot speak privately

Location requirement

You must be physically located in the state of Florida at the time of each telehealth appointment. You must notify CRS Recovery if you will be outside Florida temporarily, as we cannot provide services outside our licensed jurisdiction.

 ⚠ Controlled substance diversion

Selling, sharing, or transferring your Suboxone prescription to another person is a serious federal crime and will result in immediate termination of your treatment with CRS Recovery and may be reported to law enforcement.

Section 07

No Emergency Services

 ⚠ CRS Recovery does not provide emergency services

CRS Recovery is not an emergency medical service and cannot respond to medical or psychiatric emergencies. If you are experiencing a medical emergency, are in immediate danger, or are having thoughts of harming yourself or others — call 911 immediately or go to your nearest emergency room.
For mental health crises, call or text the 988 Suicide and Crisis Lifeline by dialing 988. For substance use disorder emergency resources, contact SAMHSA’s National Helpline at 1-800-662-4357 (free, confidential, 24/7).
 
CRS Recovery’s telehealth platform is not monitored outside of business hours. Do not use our contact form, patient portal messaging, or email to communicate urgent or emergency health concerns. Always call 911 for emergencies.
Section 08

No Guarantee of Treatment Outcomes

CRS Recovery provides evidence-based, clinically sound treatment for opioid use disorder. However, we cannot guarantee specific treatment outcomes. Recovery from opioid use disorder is an individualized process influenced by clinical, behavioral, social, and environmental factors.
 
By using our services, you acknowledge and agree that:
  • CRS Recovery does not guarantee that Suboxone treatment will result in recovery from opioid use disorder
  • Treatment outcomes vary significantly between individuals based on biological, psychological, social, and environmental factors
  • Relapse during treatment is a recognized medical event and does not constitute a failure of care
  • You are responsible for following your treatment plan, attending appointments, and taking medications as prescribed
The statistical outcomes cited on our website (such as reductions in overdose risk) are based on peer-reviewed research studies and represent population-level outcomes. They are not predictions of individual patient results.
Section 09

Limitation of Liability

To the fullest extent permitted by Florida law, CRS Recovery, its owner, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our website or services, including but not limited to:
  • Damages arising from reliance on information on this website
  • Damages arising from interruption or unavailability of telehealth services due to technical issues
  • Damages arising from changes to federal or state telehealth prescribing regulations that affect your treatment
  • Damages arising from your failure to follow your prescribed treatment plan
Nothing in these Terms of Service limits CRS Recovery’s liability for gross negligence, willful misconduct, or any liability that cannot be excluded under applicable law, including professional malpractice. CRS Recovery maintains professional liability insurance as required by Florida law.
 
This website and its content are provided “as is” without warranties of any kind, express or implied, except for the professional care obligations owed to you as a patient under Florida law.

Intellectual Property

All content on this website — including text, graphics, logos, images, the CRS Recovery name, and the design and layout of the website — is the property of Comprehensive Recovery Solutions and is protected by United States copyright and intellectual property laws.
 
You may not reproduce, copy, modify, distribute, transmit, display, publish, or sell any content from this website without the prior written consent of CRS Recovery. You may print or download content for your own personal, non-commercial use only.

Website Use & Prohibited Conduct

By using this website you agree not to:
  • Use the website for any unlawful purpose or in violation of any applicable law or regulation
  • Attempt to gain unauthorized access to any part of the website or our systems
  • Submit false, misleading, or fraudulent information through any form on this website
  • Use the website to impersonate any person or entity
  • Interfere with the proper functioning of the website or transmit any harmful code or malware
  • Use automated tools to scrape, harvest, or collect data from this website without written permission
CRS Recovery reserves the right to block or restrict access to this website for any user who violates these terms.
Section 12

Termination of Services

Termination by CRS Recovery

CRS Recovery may terminate or suspend your access to our services at any time, with or without notice, for any of the following reasons:
  • Violation of these Terms of Service
  • Providing false or fraudulent information
  • Diversion, sharing, or misuse of controlled substance prescriptions
  • Abusive, threatening, or inappropriate behavior toward our provider or staff
  • Non-payment of fees
  • Clinical determination that our services are no longer appropriate for your needs
  • Changes in law or regulation that affect our ability to provide services

Relapse — not grounds for termination

A relapse during treatment is not, by itself, grounds for discharge. CRS Recovery is committed to supporting patients through the full recovery journey. If you relapse, please contact us immediately so we can adjust your treatment plan accordingly.

Termination by patient

You may discontinue treatment with CRS Recovery at any time. We ask that you notify us so we can provide appropriate referrals and ensure your safety during any transition in care.

Obligations upon termination

Upon termination of services: all outstanding fees become immediately due and payable; we will not refill prescriptions after the termination date; and we will provide a summary of your care and referrals to other providers upon request, in accordance with HIPAA and 42 CFR Part 2.
Section 13

Governing Law & Dispute Resolution

Governing law — Florida, United States
These Terms of Service shall be governed by and construed in accordance with the laws of the State of Florida, United States of America, without regard to its conflict of law provisions. Any dispute arising from these Terms or your use of our services shall be subject to the exclusive jurisdiction of the courts of Broward County, Florida.
If any provision of these Terms of Service is found to be unenforceable by a court of competent jurisdiction, the remaining provisions shall continue in full force and effect.

Dispute resolution

Before initiating any legal proceedings, we encourage you to contact us directly at cristina@crsrecovery.com to attempt to resolve any dispute informally. Many issues can be resolved quickly through direct communication.
 
For disputes relating to your medical care or prescribing decisions, you may also file a complaint with the Florida Department of Health at flhealthsource.gov or call 1-800-342-0429.
Section 14

Changes to These Terms

CRS Recovery reserves the right to update or modify these Terms of Service at any time. We will update the “Last Updated” date at the top of this page whenever changes are made. For material changes — particularly any changes affecting patient rights, fees, or the scope of services — we will provide notice through our patient portal or direct communication.
 
Your continued use of our website or services after any changes to these Terms constitutes your acceptance of the updated Terms. If you do not agree to the updated Terms, you must discontinue use of our services.
 
We recommend reviewing these Terms periodically to stay informed of any changes, particularly given the evolving regulatory landscape for telehealth prescribing of controlled substances.
