Description of Services
- Clinical evaluation for opioid use disorder (OUD) via secure telehealth video appointments
- Prescribing of Suboxone (buprenorphine/naloxone) and other FDA-approved medications for the treatment of opioid use disorder, where clinically appropriate
- Ongoing medication management and follow-up telehealth appointments
- Supportive clinical follow-up and recovery-oriented care
- Referral coordination with other healthcare providers when clinically indicated

Eligibility Requirements
- Be 18 years of age or older
- Be physically located in the state of Florida at the time of each telehealth appointment
- Be seeking treatment for opioid use disorder
- Have access to a device with audio and video capability and a stable internet connection for telehealth appointments
- Not be experiencing an immediate psychiatric or medical emergency at the time of service
- Be willing to participate in telehealth appointments and follow your provider’s treatment recommendations
- Provide accurate, complete, and truthful information to your provider at all times
⚠ Ineligibility
CRS Recovery reserves the right to decline services to any individual who does not meet eligibility requirements, provides false or misleading information, or whose clinical needs exceed the scope of our telehealth services.

Telehealth Services & Controlled Substance Prescribing
Legal authority to prescribe via telehealth
Prescription requirements
- Prescriptions are issued only after a thorough clinical evaluation by a licensed, DEA-registered provider
- Prescriptions are issued for legitimate medical purposes in accordance with the standard of care
- Suboxone prescriptions are issued only to patients with a clinical diagnosis of opioid use disorder
- Prescription refills require ongoing participation in follow-up telehealth appointments
- CRS Recovery reserves the right to modify, suspend, or discontinue prescribing at any time based on clinical judgment

Current fees
- Initial assessment:$300 — includes clinical evaluation, diagnosis, treatment planning, and Suboxone prescription where clinically appropriate
- Ongoing monthly care:$150 per month — includes follow-up appointments, medication management, prescription refills, and dosage adjustments

Section 06
Patient Responsibilities
Accurate information
- Provide complete, accurate, and truthful information about your medical history, current medications, substance use, and any other information relevant to your care
- Notify your provider immediately of any changes to your health status, medications, or circumstances that may affect your treatment
- Never provide false information to obtain a prescription — this is a federal crime
Treatment participation
- Attend all scheduled telehealth appointments on time and fully prepared
- Take medications only as prescribed and never share, sell, or transfer your Suboxone prescription to another person
- Notify your provider if you relapse or use any substances outside your treatment plan — this information will not result in immediate discharge and is essential for your safety
- Participate in urine drug screening when requested by your provider as a standard condition of treatment
Technology requirements
- Ensure you have a reliable internet connection, a functioning camera and microphone, and a private space for your telehealth appointments
- Do not participate in telehealth appointments while driving, operating machinery, or in a public location where you cannot speak privately
Location requirement
You must be physically located in the state of Florida at the time of each telehealth appointment. You must notify CRS Recovery if you will be outside Florida temporarily, as we cannot provide services outside our licensed jurisdiction.
⚠ Controlled substance diversion
Selling, sharing, or transferring your Suboxone prescription to another person is a serious federal crime and will result in immediate termination of your treatment with CRS Recovery and may be reported to law enforcement.

No Guarantee of Treatment Outcomes
- CRS Recovery does not guarantee that Suboxone treatment will result in recovery from opioid use disorder
- Treatment outcomes vary significantly between individuals based on biological, psychological, social, and environmental factors
- Relapse during treatment is a recognized medical event and does not constitute a failure of care
- You are responsible for following your treatment plan, attending appointments, and taking medications as prescribed

- Damages arising from reliance on information on this website
- Damages arising from interruption or unavailability of telehealth services due to technical issues
- Damages arising from changes to federal or state telehealth prescribing regulations that affect your treatment
- Damages arising from your failure to follow your prescribed treatment plan

Website Use & Prohibited Conduct
- Use the website for any unlawful purpose or in violation of any applicable law or regulation
- Attempt to gain unauthorized access to any part of the website or our systems
- Submit false, misleading, or fraudulent information through any form on this website
- Use the website to impersonate any person or entity
- Interfere with the proper functioning of the website or transmit any harmful code or malware
- Use automated tools to scrape, harvest, or collect data from this website without written permission

Termination of Services
Termination by CRS Recovery
- Violation of these Terms of Service
- Providing false or fraudulent information
- Diversion, sharing, or misuse of controlled substance prescriptions
- Abusive, threatening, or inappropriate behavior toward our provider or staff
- Non-payment of fees
- Clinical determination that our services are no longer appropriate for your needs
- Changes in law or regulation that affect our ability to provide services
